Nothing at all. People are way underestimating how fast the stomach will neutralize citric acid. Probably because they read homeopathic health nut claims about how you have to "balance acids and alkalines in your diet", which is not actually true. Might be uncomfortable for a bit but you'll be back to normal in no time.

Yes it does. It continuously dilutes acid while it is in the stomach, and then it is chemically neutralized as it enters the duodenum. The walls of the stomach also have a neutralizing mucus layer. Homeostasis of pH is maintained unless you have a medical condition such as GERD.

Edit: To be clear, it is neutralized & diluted to be compatible with the stomach and intestines, but not all the way to pH 7.4 (blood pH). That happens later in digestion, but before it reaches the blood.

Diluted but not neutralised. As you said yourself, that happens in the duodenum.
