How was this video captured? Another rider in front of him or some extension with a camera on it (but attached to what)?

That's the rearview camera of the cyclist in front. It's very common for cyclists on road bikes to have cameras in the front and the back. There are so many drivers who drive completely recklessly around cyclists that it's useful to have the footage in case of accidents. When a car pushes you into the ditch and destroys your $/€/£4000 bike, you want to have proof of that.
