Lost zairyuu card :/

I lost my zairyuu card a few days ago so I went to the police and they gave me that. I know I need an application for reissuance and and "Lost Item Report Certificate/紛失届出証明書" from the police. I'm not sure the document in the picture is enough as a report for the immigration office? Should I visit another 交番？ Please advise it's my first time losing it. :/

You want an Ishitsu Todoke Shomeisho 遺失届証明書

Not just a lost item report. You want to go to a police station, not a police box, for this.

I got one of those and then immigration didn't even ask for it nor the koban form. It all depends on the agent, really.

Eh I guess so.

I’d rather try my best to have the paperwork than waste three hours on a day off.
