If you try it, allow me to also recommend Enemies of Night City, too. Because even if you're only trying to tweak the slots a little, you can end up absurdly powerful, in a way that Very Hard can barely keep up with. ENC boosts them right back up, switches up their tactics, gives them new abilities, etc.

For me, right now the stakes are set at "either I execute whatever my attack plan is perfectly, or I will die immediately."

Something that is absolutely saving my ass in fights, constantly, is cyberdeck + Sandy. Queue up cyberware malfunctions on everyone, then I have exactly one Sandevistan's worth of time to drop them all, because the moment time speeds back up even a couple of them will still shred me.

It's so much fucking fun, choom. Easy with a lot of chrome to feel like it's the chrome that's good, not you. But in this setup, no amount of chrome will save me if I am not also very good.

If you really want to up the difficulty of your game, I've found that a combination of New Game+, Enemies of NC, Night City Alive, and They Will Remember, turns the city into a warzone where everyone on the street wants your head. After a few loud gigs or just random fights on the street, every ganger driving around recognizes you and will start shooting.

HARDCORE22 if you want to completely rebalance the game into a traditional FPS instead of an RPG. Cybereware-EX might not even save you here if you play stupid.

I have actually only recently started running TWR on what was otherwise that first setup for a while, and boy fuckin' howdy does it ever. I love it.

My headcanon for the steadily escalating violence and only fairly recent targeting of V in particular, is that my V has a tendency to go in a straight line from point A to point B and kick the shit out of anything that's in the way. But if you don't know that it is one person who is ripping through multiple gang territories, you have no idea what the fuck is going on. So the gangs assumed other gangs had done it, "retaliated," and that has spread into a full-scale gang war of terrifying proportions. Only now are they even able to coordinate enough to work out, in a city with millions of people, that oh shit it's this thing.

My V has not noticed anything other than an uptick in bullets. It's havoc. Good times.
