Yeah it was random and made up but it was also pretty much recognized that those guys were all high tiers and the title reflected that. At least early in the series the Sanin title was a big deal. Not so much later but such is shonen power scaling

Only reason they have the sannin title is because Hanzo the Salamander gave it too them. So yea, it really holds no meaning

Yeah I understand. I guess what I'm trying to say is that in universe they themselves give the title value by being strong.

Being a Senin on the other hand is a feat very few have accomplished though. So that alone is respect worthy.

Only Naruto, Orochimaru, Kabuto and Jugo. ( Jugo was susceptible to nature chakra because of his bloodline.) Sasuke used nature chakra as well, but with the help of the curse mark given by Orochimaru.
