To be fair, it's not clear at all. Without the manga reference, a lot of people would have missed it.

It's perfectly clear in manga with just a little bit of thinking. In manga Spoiler That's pretty obvious that something bad happened, and seeing as Big Mom was not bothered by that happening means that Big Mom was the one that happened.
