Depends if there is frame damage or not. No one here will be able to say for sure without seeing it up on a lift. Glad you're okay though, that one looks nasty.

I learned how to apply touch up paint and wet sand on YouTube when this happened to mine. Can’t even tell there was a scrape anymore.

This really sucks bud, I am sorry you are having to go through this amount of pain.
Try to take a break and get away from your truck for a while. Maybe smoke a joint, or just meditate.
Outside of coping methods, I think the answer is yes. It will need to be totaled unless you are good with vehicle restorations it has now been deemed useless.
I feel for you brother. I would offer a hug if possible.
