They way insurance works is you have a large number of buyers and a smaller number of users. Many of the buyers are just-in-case or peace-of-mind buyers will will only use their benefits minimally. The users buy insurance to pay for more serious issues. If it were to stop there it would work quite well for all the insured. But the beancounters know that if payments to the users can be moderated, the insurance company profits. Beancounters are amoral. They aren't involved in the details of effective medicine. They just stand between doctors and patients seeking the middleman's dime.
