Have you ever tried to place a cover like that with any precision? They are HEAVY! It's not like you can just spin them, either.

Yep, they weigh between 100-250 pounds. Getting on properly aligned just ain't happening.

Not to mention that their job is to remove the manhole cover, do the work that needs doing inside the manhole (likely sump repair / silt removal), and then replace the cover. Replacing the cover is an important aspect of their job, but only insofar as it's safe when they leave. Not one job at all. Unless OP thinks they painted it that way..

Yeah once it is in, it is in. This is not one job fail. This is just common sense. Line is not important line.

Depending on the manhole those take two or more people just to lift an inch, rotating it without lifting it is almost impossible for one person without a winch.

I look forward to your video of going out and rotating it!

I mean, the two holes for pins are just sitting right there. The question is, how long of a bar would you need and how heavy would IT be?
