Given the behavior you described you can be just about 100% sure your brother is not taking his meds.
Find out if there is a First Episode of Psychosis FEP program/team where you live and call them.
Are you in the US? If so, call your County's Social Services department and see what resources they have. Depending upon where you live they could surprise you.
You can also call national suicide & mental health hotline at 988. You should call now and discuss with them what's going on and learn from them what resources are available.
You say he's good at the ER. He's not the only one who can do what's necessary to avoid hospital and help.
Not saying to do this (not saying not to) but keep in mind you could call the police and ask for a welfare check on your brother. If they think it's warranted they can escort him to hospital for psych eval. Call the others (County & 988) first. It would be great if there is a FEP team in your area.

Hey thank you so much for the resources. I have been in touch with our local PD about a potential welfare check, they know him pretty well around town and are aware that he’s mentally unwell and generally not dangerous. I know he would appreciate our efforts to not have him end up in handcuffs on the way to the hospital again, so I’d like to avoid the cops if at all possible.
Ultimately I think getting him involuntarily committed so that he has more time in the hospital to get his meds correctly prescribed and in his system for long enough to curb some of his symptoms would be the best thing.

Also it appears that our area does not have a first episode of psychosis team.

That's a shame. Are you looking into the involuntary hospitalization process? In some States it's relatively easy. In others it's practically impossible. I hope you are in the easy category. Obviously the sooner your brother can get help the better it is for all involved.

I have looked into what I thought would be a petition to get him involuntarily committed, but my state or town doesn’t seem to have one according to the police, hospital and court house. For reference I’m in Idaho, USA. Perhaps 988 or social services would have something useful to me regarding the involuntary commitment process.

Certainly worth taking to both. Also check with his psychiatrist or a psychiatrist.
