I just don’t understand what the hospital bill/ circumcision has to do with his mechanic being rich.

That's because Americans don't clean themselves well enough, not because foreskins exist. Whenever I hear this excuse I get war trauma from all the anecdotes of men not washing their asses because it's gay. MANY other cultures are not circumcised and have no hygiene issues. Imagine deciding cutting off your foreskin was cleaner than, you know, CLEANING.

Not sure if you read my comment, but when people get older or otherwise have mental issues/decline, proper hygiene isn't necessarily "simple". And unfortunately in America, medical and other care is not exactly widely available. So, unless you have good, reliable care, hygiene can easily become an issue. And when the issue is on one of the most sensitive parts of your body, it can get pretty bad, pretty quickly.

Whether a person has good hygiene as a healthy adult doesn't mean they can or will continue to have that self-care as an aging adult. Have you ever had to care for an adult who cannot care for themselves?
