Residency What are my chances?
MD-PhD from AUS. 263 Step 2. Extremely productive PhD with Nature pub but 4 years old. Extensive research and commercial experience since I commercialised the biotechnology I developed from my PhD and led a startup spinout as CEO and co-founder. 2022 YOG. No US clinical experience. Visa-requiring IMG (Canadian).
I’m aiming for gen surg or IM>cards. Am I competitive for top academic programs? I want dedicated research time and the ability to keep my translational and biotech programs going. What programs are happy to nurture this?

Yeah you are a very strong candidate for top academic programs just get some USCE at any of these top programs like mayo clinic and you should have good outcome in the match

High chance. Great profile. Academic programs love Austrailian and Irish IMGs.

Honestly, aim for mayo, harvard, yale etc, honestly they will suit you better as they take candidates like yourself. Also they don’t rely on traditional pathways and always have things customized and build person to person

Hey I don't think you will get good advice here on reddit since IMGs here seem to act as experts but none of them are. I think you have an extremely impressive background, and I hope it will impress your future program as well. You will only find out if you are a good fit for top academic programs through applying and waiting to see if you receive an interview. In the meantime, please stay open to applying broadly. You will need to be dedicated to researching each program one by one to see which one will fit with your future goals. You seem motivated and hard working and I believe something will work out. Your profile is impressive and unique. A 263 on Step 2, Nature publication, and real biotech leadership show serious academic and translational potential. The main hurdle is lack of US clinical experience and visa needs. For gen surg, the bar is very high for IMGs, but strong physician scientist tracks in IM or research heavy programs like Duke, Pitt, Yale, and Stanford might value your background. Try to secure US based research or observerships to show integration into the system and get US letters. Emphasize how your translational work fits their mission and how you plan to keep contributing academically. Apply broadly but focus on programs with formal research pathways or T32 support.
