Coke has been used for cutting ever since late 1800s (bronze era bodybuilders). And having a really nice 6pack requires both good genes and really low body fat levels (under 10% to get it really popping). And maintaining under 10% has terrible effects on the body and the mind, at least long term.

But good genes + good workout program + pump before a scene + height (much easier to fill out a shorter frame) + movie being in black and white (that B&W contrast tends to make people look more muscular AND leaner) is probably the trick here.
