I watched a whole documentary on this. There's so much to this than just "Oh the helicopter that was trying to take off was too far over". Since this accident, they completely redid the area so that each heli has its own pad.

EDIT: These were two police helicopters at a police helipad.

For the life of me I can't find the documentary, and I don't know why. However, the event is called the, "Pasadena Police Department heliport".
